Home Care Services

Call on us for care you can trust, in the comfort of your own home. VNHC can help make the transition from hospital or rehab center to home as easy as possible. We’re here when you need us, 24 hours a day. VNHC home care  services include:

Skilled Nursing:  Our highly professional and experienced team of nurses can help you recover from surgery or recent illness.  Your nurse will coordinate all aspects of your care and communicate regularly with your physician.  Our nurses are skilled in disease management, wound care, post-surgical care, pain management and family education.

Telemonitors:  For patients whose health is unstable or who have suffered frequent hospitalizations (i.e. congestive heart failure or COPD), telemonitors are available to help us monitor your vital signs closely.  This gives patients a greater sense of security and avoids re-hospitalization.

Therapy Services:  If therapy services have been prescribed for you at home, we offer physical therapy, occupational therapy and speech therapy.

Home Health Aide Services:  When you need help with daily activities such as bathing, dressing or feeding.

Medical Social Work:  If additional resources are required, our social work team can provide you with information, serve as a liaison to other community agencies, and help you with issues beyond your physical needs.

Private Duty Aides:  We offer assistants on an hourly basis to help elderly patients manage their daily lives more easily.  Aides can provide light housekeeping, errands, meal preparation, grooming and companionship.

Payment and Insurance Plans

VNHC accepts payment for services from Medicare and Medicaid programs, from private insurance plans and managed care plans. Please call our intake department at 203-276-3004 if you need assistance with coverage.
